I got all of mine from Chessex. They charge $1 per die, but you have to get 10 of each design. I would suggest rallying a bunch of people together to get them cheaper, but the WB seems to disagree with this tactic.

On their website. They have a form that you email in. The customer service is really good and you can talk with the rep to help make sure you get what you want. The whole company is just 3 people, so making 6k+ custom dice was pretty much nuts for them, so I recommend not doing that =P

It can be either the "1" or the "6", you just specify. Most people get the symbol as the "6".

Also, either Marvel or DC (or maybe both) issued a cease and desist, as those dice aren't licensed, so I don't think Chessex (I assume it was them that mas making those larger orders) will be making anymore.

There is talk of getting non-copyrighted dice made. For example, a lightning bolt can be for any Flash, electrical based, or Marvel Family team. A white military star on a blue die could be for Captain America. "Twip" (I don't think that's copyrighted) would be for Spidey (of course, you could just use a generic red spider on a blue die too).
