Nope, Leap/Climb and Phasing characters NEVER EVER roll to break away

No, characters with phasing (and Leap/Climb or Hypersonic Speed) never need to make rolls to break away. They are always sucessful.

I'm not sure what your question is but let's give it a spin.
Whenever a fig moves (not carried or thrown) you give it a token.
Whenever a fig attacks you give it a token.
Whenever a fig uses a power consult your PAC (powers and abilities card) and it will tell you if it requires an action to use and whether you need to give the fig a token.
Certain powers allow you to do multiple acions at a time in such a case (i.e. flurry, Charge, Hypersonic speed) you only give your fig ONE token
Basically htere is no difference between an action token and a movement token.
