I had to DQ myself after discovering I was using the wrong Superman base and was way overbuilt. Silly part was I didn't even realize it until the 2nd match. After that, I really don't mind the loss of flight bases...
There was one time I was playing in a sealed booster (or draft, I forget which). This was around the time of Ultimates. I was playing one of the judge's sons. His team was slaughtering me. So, at one point, I started counting points to see how I could possibly pull out a win. And that's when I realized he had overbuilt by around 90 points (in like a 300 point game). I understood what had happened: he had probably had two figures that he had a hard time choosing between when making his team and then forgot that he had to choose between them and ended up with them both on his team.
Well, I called over the judge and asked what we were supposed to do. The judge gave me a win for that game with full points (and gave his son 0 points). The judge's son wasn't actually disqualified from the tournament though; he was allowed to drop a character and keep playing and he ended up in second place, if I recall correctly.
That's the closest I've got to a disqualification story.

I have come so close to doing that so many times... I haven't played sealed in a couple of years, but I used to recheck my sealed team three or four times and still found that they were at funky point values because I picked up figures that weren't on the team. I guess I can remember who belongs on a team much more easily if I took the time to hand-pick all of the figures, rather than having them basically chosen for me by random distribution.

I have never been DQ'ed. I have never seen anyone DQ'ed at a venue I've played at, and I've never DQ'ed anyone while I have been judging.
But... I've dominated people so bad that I've had many people throw their dice, throw their clicks, break their clicks, never come back to the venue, or just outright give up on the game.
And... I usually don't lose my temper..., I'm a very laid back, helpful, and friendly player, and I win fellowship a lot. But there have been a few rare times where I have begun to lose my cool (especially when playing against a few people I really can't stand. When this happens, I usually get real quiet and just attempt to decimate my opponent. Sometimes I win, sometimes I lose in these situations. But I've only actually lost my temper once, and that was when I was playing a guy who wanted to argue how Energy Explosion worked. I pulled out a printout of a discussion I had with Harpua in the Rules forums, showed him that, and called the judge over to settle the dispute. The player then argued with the judge for 5 minutes until he just gave up the argument. I then continued to wipe his entire team, and I didn't lose one figure.
But I have seen people banned from venues. Quite a few actually. Some where young kids caught stealing. Some were judges caught selling prize support.
I once lost a game and while calculating victory points realized I was over the build total. Not really much to do because I had already lost but I consider it a DQ anyway. Or maybe I won and was DQed anyway. I don't remember.
Being a bit over points is the only case I've ever seen anyone disqualified. It happens. Everyone makes mistakes* and sometimes that mistake is a miscalculation in point total. It's frustrating to everyone but we move on and are more careful in the future.

this is one aspect of the game that truly perplexes me....do most judges award prize support based on good sportsmanship? i ask because the venue i play at seems to choose mystical, dark magikal reasons. like if someone is losing horribly or they are a close friend or random dice rolls etc. havent seen a judge in my neighborhood award fellowship based on sportsmanship in a looooong time. and to be honest ive seen more corrupt judges than i have players unfortunately.
I think the judge ended up seeing what this guy was about...after his "outage" to everyone(thank you scorned girlfriend!) I never caught flak for it again...his deal was that if i painted it then I had to have modded the dial too....
Considering that the other guy ended up being a dial-changer, this reminds me of unfaithful people in relationships who are always accusing their partner of cheating on them. Projection of one's own failings...

Ask Groovy to tell the story how he helped DQ an opponent in a 3 way match against Fin Fang Foom @ WW Philly. The guy was overbuilt by 3 points.
Total d i c k move on my part, but the guy was playing a V Hyperion TK yo-yo and talking tough, so I was like, "Is there anything I can do to at least make the next 50 minutes moderately enjoyable?"

I was DQ'ed in one of the qualifiers for the Wizard World Championships (the ones you play locally, not when you're at the con). I overbuilt and didn't notice until I had a full wipe of my opponent in the second or third round. So I blew the whistle on myself and all my dreams of winning a legitimate glass trophy came crashing down...
